This week we review some key ophthalmologic findings with excellent teaching images for you to see. We also hit the lung, the joints, the extremities, and even spend time on scorpions, snakes, and spiders.&nbsp;<\/h6>\n\n\n\n<hr class=\"wp-block-separator\" \/>\n\n\n\n<p>We begin each episode with a rapid review of the previous week&#8217;s highlights then dive into the Q&amp;A and wrap up with another rapid review of the covered content. Let&#8217;s get started.<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"201\" height=\"74\" src=\"https:\/\/www.roshreview.com\/wp-content\/uploads\/RapidReview-2-e1475624722961.png\" alt=\"\" class=\"wp-image-818\" \/><\/figure><\/div>\n\n\n\n<ul class=\"wp-block-list\"><li>The <strong>most common cause of pneumonia<\/strong> in an <strong>alcoholic<\/strong> is <em><strong>Streptococcus pneumoniae<\/strong><\/em>.<\/li><li><strong>Radial nerve palsy<\/strong> is a peripheral nerve lesion caused by compression of the radial nerve in the axilla. The most common findings are wrist drop and hand numbness over the first to third digits. It is managed conservatively with a wrist splint.<\/li><li>IV drug abusers are at risk for<strong> tricuspid endocarditis<\/strong>. The most commonly implicated organism is staph aureus.<\/li><li><strong>Colchicine overdoses<\/strong> cause severe GI distress, bone marrow suppression, rhabdomyolysis, renal failure, metabolic acidosis, and even ARDS.<\/li><li>The mainstay of treatment for <strong>herpes zoster<\/strong> is pain control. Acyclovir may be prescribed within the first 72 hours of onset of the lesions.<\/li><\/ul>\n\n\n\n<p><strong>Now onto this week\u2019s podcast<\/strong><\/p>\n\n\n\n<hr class=\"wp-block-separator\" \/>\n\n\n\n<p><strong>Question 1<\/strong><\/p>\n\n\n\n<p>A 43-year-old man is currently undergoing palliative radiation therapy for pancreatic cancer. He presents to the emergency department with a 4-day history of gradually worsening left lower extremity pain. Pain is localized to the calf and made worse with ambulation. Exam shows mild edema of the left calf without erythema or warmth. Which of the following is the most likely diagnosis?<\/p>\n\n\n\n<p><span>A. Cellulitis<\/span><\/p>\n\n\n\n<p><span>B. Deep venous thrombosis<\/span><\/p>\n\n\n\n<p><span>C. Peripheral artery disease<\/span><\/p>\n\n\n\n<p><span>D. Sputum<\/span><\/p>\n\n\n\n<hr class=\"wp-block-separator\" \/>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"201\" height=\"74\" src=\"https:\/\/www.roshreview.com\/wp-content\/uploads\/RapidReview-2-e1475624722961.png\" alt=\"\" class=\"wp-image-818\" \/><\/figure><\/div>\n\n\n\n<ul class=\"wp-block-list\"><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/dvts\" target=\"_blank\">DVTs<\/a><\/strong> typically present with non-specific symptoms including unilateral limb swelling, pain, redness, warm, or even distended collateral veins.<\/li><li>Predisposing <strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/risk-factors-for-dvt\" target=\"_blank\">risk factors for DVT<\/a><\/strong>&nbsp;include malignancy, immobilization, recent surgery, obesity, smoking, oral contraceptives, recreational drugs, and hypercoagulable states.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/corneal-abrasions\" target=\"_blank\">Corneal abrasions<\/a><\/strong> typically resolve within 24-72 hours. They are most commonly caused by ocular trauma or an ocular foreign body. Corneal abrasions are treated with a topical antibiotic such as erythromycin or ciprofloxacin. Tetanus vaccination should be given to patients who are not up to date.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/allopurinol\" target=\"_blank\">Allopurinol<\/a><\/strong> is contraindicated in an acute presentation of <strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/gout\" target=\"_blank\">gout<\/a><\/strong>. The first-line therapy is NSAIDs.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/negatively-birefringent-crystals\" target=\"_blank\">Negatively birefringent crystals<\/a><\/strong> are seen in <strong>gout<\/strong> and <strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/positively-birefringent-crystals\" target=\"_blank\">positively birefringent crystals<\/a><\/strong>&nbsp;are seen in <strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/pseudogout\" target=\"_blank\">pseudogout<\/a><\/strong>. Gout is typically seen in the first metatarsophalangeal joint, whereas pseudogout most commonly affects the larger joints like the knees and ankles.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/acute-glaucoma\" target=\"_blank\">Acute glaucoma<\/a> <\/strong>often classical presents as a red, painful eye with blurry vision and asymmetric pupils.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/acute-angle-closure-glaucoma\" target=\"_blank\">Acute angle-closure glaucoma<\/a><\/strong> is caused by closure of the anterior chamber. Treatment should begin with topical beta-blockers, carbonic anhydrase inhibitors, steroids, and miotics.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/scorpion-stings\" target=\"_blank\">Scorpion stings<\/a><\/strong> produce local redness and edema with a heightened sensitivity to touch in the area of the sting with numbness and weakness of the affected area. Systemic symptoms include fasciculations, disconjugate gaze, temperature reversal, and pancreatitis.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/black-widow-spider-bites\" target=\"_blank\">Black widow spider bites<\/a><\/strong> cause a local papule with a halo. Severe systemic symptoms include a peritonitic abdomen, muscle fasciculation\u2019s, and diaphoresis. Black widow spiders have a red hourglass on their abdomen and can be found throughout the entire United States.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/brown-recluse-spiders\" target=\"_blank\">Brown recluse spiders<\/a> <\/strong>are found in the Midwest. They cause a papule that later blisters and may necrose. Systemic symptoms include renal failure, pulmonary edema, and shock.<\/li><li><strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/pit-viper-snakebites\" target=\"_blank\">Pit viper snakebites<\/a><\/strong> cause local swelling and oozing from the wound. Severe envenomations can lead to a DIC-like coagulopathy and hemorrhagic bullae.<\/li><li>The most common symptom of <strong><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"\/tag\/acute-bronchitis\" target=\"_blank\">acute bronchitis<\/a><\/strong>&nbsp;is a <strong><a href=\"\/tag\/cough\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">cough<\/a><\/strong>. The most common cause is a viral illness that should be treated with supportive care.<\/li><\/ul>\n\n\n\n<hr class=\"wp-block-separator\" \/>\n\n\n\n<p>Need to beef up on your colchicine toxicity knowledge?
