We focused on a few high-yield bites and envenomation. This material is easy to gloss over since we do not come across it too often clinically. Not only will this prepare you at the bedside, but it will also help you pick up a few easy points on the In-Training Exam, which, by the way, is right around the corner.<\/h6>\n\n\n\n<hr class=\"wp-block-separator\" \/>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"201\" height=\"74\" src=\"https:\/\/www.roshreview.com\/wp-content\/uploads\/RapidReview-2-e1475624722961.png\" alt=\"\" class=\"wp-image-818\" \/><\/figure><\/div>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Envenomation<\/strong> by a <strong>Portuguese Man O\u2019 War<\/strong> is treated by immediate <strong>removal of the nematocyst<\/strong> followed by a <strong>salt-water wash<\/strong>.<\/li><li><strong>Black widow spiders<\/strong> have a <strong>red hourglass<\/strong> on their abdomen.<\/li><li><strong>Bites<\/strong> from a <strong>black widow spider<\/strong> produce a <strong>local papule with a halo<\/strong>. Severe symptoms include <strong>muscle fasciculations<\/strong> and a <strong>peritonitic abdomen<\/strong>.<\/li><li><strong>Brown recluse spider bites<\/strong> cause a <strong>papule that blisters and then necroses<\/strong>. Severe symptoms include <strong>renal failure, pulmonary edema, and shock<\/strong>.<\/li><li>A <strong>pit viper snake<\/strong> envenomation causes <strong>local swelling and oozing<\/strong> from the wound. Severe envenomation can cause a <strong>DIC-like coagulopathy with hemorrhagic bullae<\/strong>.<\/li><\/ul>\n\n\n\n<p><strong>Now onto this week\u2019s podcast!<\/strong><\/p>\n\n\n\n<hr class=\"wp-block-separator\" \/>\n\n\n\n<p><strong>Question 1<\/strong><\/p>\n\n\n\n<p>A 4-year-old boy is brought to the Emergency Department after a drowning. Bystanders noted the child was underwater for less than a minute. After quickly being pulled up from the bottom of the pool, he coughed and vomited once. He arrived at the ED awake and alert with a GCS 15. On examination, he is afebrile with heart rate 105 beats\/minute, respiratory rate 20 breaths\/minute, and oxygen saturation 96 % on room air. His lungs are clear to auscultation. What is the next step in management?<\/p>\n\n\n\n<p><span>A. Admit to telemetry unit for 24-hour observation<\/span><\/p>\n\n\n\n<p><span>B. Treponema pallidum<\/em><\/span><\/p>\n\n\n\n<p><a href=\"Image - penile lesions, chancroid, syphilis, granuloma inguinale, lymphogranuloma venereum\">Teaching image<\/a><\/p>\n\n\n\n<hr class=\"wp-block-separator\" \/>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"201\" height=\"74\" src=\"https:\/\/www.roshreview.com\/wp-content\/uploads\/RapidReview-2-e1475624722961.png\" alt=\"\" class=\"wp-image-818\" \/><\/figure><\/div>\n\n\n\n<ul class=\"wp-block-list\"><li>When caring for a <strong>drowning<\/strong> victim with normal vitals and abnormal exam, a <strong>4 to 6 hour observation period<\/strong> is sufficient before discharge. Any <strong>oxygen requirement<\/strong> or pulmonary finding on exam <strong>requires admission<\/strong>.<\/li><li><strong>Elderly patients<\/strong> with <strong>abdominal pain<\/strong> are more likely than younger patients to require an <strong>emergent surgical procedure<\/strong>.<\/li><li>The two most common causes of <strong>SBOs<\/strong> are <strong>adhesions<\/strong> followed by <strong>cancer<\/strong>. &nbsp;<\/li><li><strong>Large bowel obstructions<\/strong> are caused most commonly by <strong>cancer<\/strong> followed by <strong>volvulus<\/strong>.<\/li><li><strong>Mean arterial pressure<\/strong> is defined as Cardiac Output x Systemic Vascular Resistance + Central Venous Pressure. It is often approximated by <strong>two-thirds of the diastolic<\/strong> blood pressure <strong>plus one-third of the systolic<\/strong> pressure.<\/li><li><strong>Myocardial contusions<\/strong> are a sequela of <strong>blunt chest trauma<\/strong>. &nbsp;The most common EKG finding is <strong>sinus tachycardia<\/strong>. These patients require an <strong>echocardiogram<\/strong>. The most common course is spontaneous resorption of resolution of the symptoms. The most serious complication is <strong>delayed rupture<\/strong>.<\/li><li>The <strong>hypoxemia<\/strong> of <strong>right-to-left<\/strong> shunt would not improve substantially with supplemental oxygen and the <strong>A-a gradient would be increased<\/strong>.<\/li><li><strong>Diffusion impairment<\/strong> occurs as a result of <strong>interstitial disease<\/strong> which <strong>improves with supplemental O2<\/strong> and has a large A-a gradient<\/li><li><strong>Hypoventilation<\/strong> occurs during drug<strong> overdoses, CNS lesions, and neuromuscular junction disorders<\/strong>. &nbsp;Hypoxemia in this setting <strong>improves with supplemental oxygen<\/strong>, and the A-a gradient is usually normal.<\/li><li>In V-<strong>Q mismatches, hypoxemia improves with supplemental oxygen<\/strong>, and there is an increased A-a gradient.<\/li><li>There are three <strong>painless<\/strong> <strong>penile lesions<\/strong> to remember: &nbsp;The <strong>small, shallow ulcer of<\/strong> <strong>LGV<\/strong> caused by <strong><em>Chlamydia<\/em><\/strong>. &nbsp;The <strong>beefy red ulcer<\/strong> and painless papule of <strong>Granuloma Inguinale <\/strong>caused by <strong><em>Klebsiella<\/em><\/strong>. Lastly the painless <strong>chancre of Syphilis<\/strong> caused by <strong><em>Treponema pallidum<\/em><\/strong>.<\/li><li>The two <strong>painful penile lesions<\/strong> are the multiple painful papules that ulcerate of <strong>Chancroid<\/strong> caused by <strong><em>H. ducreyi<\/em><\/strong> and the shallow tender painful lesions of <strong>Genital Herpes<\/strong> caused by <strong>HSV<\/strong>.<\/li><\/ul>\n\n\n\n<p>Hope you enjoyed this week\u2019s episode.
