If so, you\u2019re not alone. Choosing a specialty in medical school isn\u2019t easy. You\u2019re required to make a career-long decision from a series of brief rotations, each spanning a couple of weeks to months and then hope you made the right choice.<\/h6>\n\n\n\n<h6 class=\"wp-block-heading\" id=\"h-most-of-the-time-this-works-out-and-your-career-path-becomes-everything-you-wished-for-but-sometimes-maybe-more-often-than-you-might-think-it-doesn-t-if-you-find-yourself-in-this-second-camp-and-wonder-whether-the-trajectory-you-re-on-is-the-right-one-here-are-some-things-to-know-about-switching-specialties-during-residency-and-what-steps-you-need-to-take-nbsp\">Most of the time, this works out, and your career path becomes everything you wished for. But sometimes\u2014maybe more often than you might think\u2014it doesn\u2019t. If you find yourself in this second camp and wonder whether the trajectory you\u2019re on is the right one, here are some things to know about switching specialties during residency and what steps you need to take.&nbsp;<\/h6>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity is-style-wide\"\/>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-1-identify-what-s-motivating-these-feelings\"><strong>1. Identify what\u2019s motivating these feelings.<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">It\u2019s important to reemphasize how challenging it is to <a href=\"https:\/\/www.roshreview.com\/blog\/choose-medical-specialty-fourth-year-medical-student-take-step-back\/\">pick a medical field<\/a> and that you&#8217;ve done nothing wrong if you&#8217;re now considering changing specialties during residency. The process of rotating through every specialty in medical school is long, and for any given specialty, you likely only interacted with a select group of individuals for a limited period of time.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Furthermore, it\u2019s natural to think about changing specialties during residency if you didn\u2019t have the opportunity to spend time in certain fields like radiology, ophthalmology, or dermatology, so you could understand what a career in these specialties is like. Once you start training, you (of course) learn more about your own field, but also about others. You naturally may wonder if yours really is the best choice for you.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you come to the conclusion that the path you\u2019re on <a href=\"https:\/\/www.roshreview.com\/blog\/6-resources-for-making-a-career-transition-in-medicine\/\">isn\u2019t what you wanted<\/a>, take a moment to breathe. Ask yourself if your discontent would actually be resolved by switching to a residency program in another specialty. <strong>Think about what you like and don\u2019t like about your specialty and how long you will have to endure the less palatable parts.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Keep in mind, even though residency is hard, <strong>it doesn\u2019t last forever. <\/strong>This doesn\u2019t mean that being an attending is easy, but it does mean that your day-to-day will change as you advance through your training and it may morph into the job you hoped for from the beginning.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-2-consider-the-career-you-truly-want\"><strong>2. Consider the career you truly want.<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">If you\u2019ve looked into the future and still have doubts about your field, reflect on what you do want with insights you\u2019ve gained over time. Some questions to help guide your deliberation about changing specialties during residency include the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Are there specific areas of medicine or patient populations that you have a <strong>particular interest<\/strong> in?<\/li>\n\n\n\n<li>Do you gravitate towards more or less <strong>acuity<\/strong>?<\/li>\n\n\n\n<li>Do you like to work in <strong>inpatient or outpatient<\/strong> settings?<\/li>\n\n\n\n<li>Which do you prefer: <strong>procedural<\/strong> work, <strong>cognitive<\/strong> work, or a mix of both?<\/li>\n\n\n\n<li>Is your <strong>work-life balance<\/strong> sufficient, or do you need more time to spend time with loved ones?<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Another option:<strong> you don\u2019t even have to stay in medicine. <\/strong>Many <a href=\"https:\/\/www.roshreview.com\/blog\/9-great-non-clinical-physician-jobs\/\">non-clinical jobs<\/a> exist for physicians. They may not have been what you set out to do when you started medical school, but they can be fulfilling all the same. The point is, there are many different roads you can take.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-3-talk-to-your-family-friends-and-mentors\"><strong>3. Talk to your family, friends, and mentors.<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">This might be the most important thing to do if you\u2019re thinking about changing specialties during residency. Even though you\u2019re the only person who can answer the question of what will make you happy, it\u2019s not easy\u2014you can (and really should) ask for help.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Talk to the people around you, including trusted family, friends, and mentors if you\u2019re thinking about changing specialties during residency. Being able to discuss what you want to do for your life\u2019s work can be validating, help combat feelings of guilt, and provide perspective.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-4-evaluate-your-options\"><strong>4. Evaluate your options.<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Once you settle on a new course, the final step is investigating the opportunities available to you. While not abundant, <strong>advanced training positions do exist for those switching to residency programs in different specialties.<\/strong> Engage with your mentors about how to identify openings (since they may not be advertised) and find out how to connect with those programs. You will likely be asked for a CV and cover letter detailing your narrative and why you\u2019re switching programs. Hopefully, you\u2019ll be invited for an interview with the program leadership and then accepted into your new job.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You should know you may have to be flexible. First, depending on how far into your training you are when making the switch, <strong>you might have to repeat a year or multiple years of residency since you are starting anew in a different field. <\/strong>Of note, internal medicine intern years are standard across many fields, so you may not have to add any additional time if you interchange during this first year of residency among specialties that require one.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Second, options in your new specialty of choice may not be available in the city where you live, so if you end up switching residency programs <strong>you may be asked to relocate to a different city.<\/strong><\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-5-once-you-make-the-decision-to-change-specialties-stay-confident-nbsp\"><strong>5. Once you make the decision to change specialties, stay confident.&nbsp;<\/strong><\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">You control your future. If you need to change course, you should do so without feeling guilty or worrying about disappointing others. From there, he attended medical school at the Perelman School of Medicine at the University of Pennsylvania in Philadelphia, PA, where he was involved in the Medical Student Government, Dermatology Interest Group, and University City community clinic. He completed a preliminary internship in Internal Medicine at Brigham and Women's Hospital in Boston, MA and is currently a dermatology resident in the Harvard combined program. Outside of medicine, Mike enjoys hiking, playing tennis, and just generally being outside.
