{"id":2222,"date":"2018-01-25T10:19:42","date_gmt":"2018-01-25T15:19:42","guid":{"rendered":"https:\/\/www.roshreview.com\/?p=2222"},"modified":"2023-03-27T13:18:29","modified_gmt":"2023-03-27T13:18:29","slug":"choose-medical-specialty-fourth-year-medical-student-take-step-back","status":"publish","type":"post","link":"https:\/\/www.roshreview.com\/blog\/choose-medical-specialty-fourth-year-medical-student-take-step-back\/","title":{"rendered":"How to Self-Reflect and Choose Your Medical Specialty This Year"},"content":{"rendered":"\n<h6 class=\"wp-block-heading\"><span style=\"font-weight: 400\">\u201cKeep your minds open,\u201d the dean announced at M3 orientation, \u201cMaybe you\u2019ve always dreamed of becoming an orthopedic surgeon but will fall in love with psychiatry.\u201d <\/span><\/h6>\n\n\n\n<hr class=\"wp-block-separator has-css-opacity\"\/>\n\n\n\n<p><span style=\"font-weight: 400\">As freshly minted third-year medical students with wrinkle-free and yet-to-be coffee\/pen\/bodily fluid-stained short white coats, we entered clinical rotations much like undifferentiated cells, <strong>eager to be shaped and influenced<\/strong> as we transformed into the future physicians we were to become. However, for many students, choosing a specialty is not as easy as <\/span><i><span style=\"font-weight: 400\"><em>dreaming<\/em><\/span><\/i><span style=\"font-weight: 400\"> and <\/span><i><span style=\"font-weight: 400\"><em>falling in love<\/em><\/span><\/i><span style=\"font-weight: 400\">. There is a fine line three-quarters into M3 year when the reaction to uncertainty about choosing a specialty changes from a response of \u201cyou\u2019ve got time\u201d to a reaction that may make you feel like somehow over a few short months you became defective. In an attempt to guide students, one such algorithm was published in an article featured in the British Medical Journal by Dr. Boris Veysman.<\/span><\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter is-resized\"><a href=\"http:\/\/www.bmj.com\/content\/331\/7531\/1529\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.roshreview.com\/wp-content\/uploads\/F1.large_-1024x973.jpg\" alt=\"Medical specialty algorithm\" class=\"wp-image-2223\" width=\"512\" height=\"487\" srcset=\"https:\/\/www.roshreview.com\/wp-content\/uploads\/sites\/2\/F1.large_-1024x973.jpg 1024w, https:\/\/www.roshreview.com\/wp-content\/uploads\/sites\/2\/F1.large_-300x285.jpg 300w, https:\/\/www.roshreview.com\/wp-content\/uploads\/sites\/2\/F1.large_-600x570.jpg 600w, https:\/\/www.roshreview.com\/wp-content\/uploads\/sites\/2\/F1.large_.jpg 1280w\" sizes=\"auto, (max-width: 512px) 100vw, 512px\" \/><\/a><figcaption class=\"wp-element-caption\">Veysman B. Physician, know thyself. <em>BMJ<\/em>. 2005;331:1529.<\/figcaption><\/figure>\n<\/div>\n\n\n<p><strong>Unfortunately, despite the color-coded flow chart, it\u2019s not that simple<\/strong>.<\/p>\n\n\n\n<p><span style=\"font-weight: 400\">One resource that may help you think more holistically about yourself within the specialties is through the AAMC Careers in Medicine website, where you can find a <a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"https:\/\/www.aamc.org\/cim\/specialty\/understandyourself\/assessments\/interests\/\" target=\"_blank\">Medical Specialty Preference Inventory<\/a>. <strong>After answering 150 questions<\/strong>,<strong> you receive an assessment with the probability that you will enter into 1 of 16 specialties based on your interest in performing specific activities in medicine<\/strong>. You can compare your \u201cMedical Interest Scale\u201d to the typical range within the individual specialties. This information can provide insight into where you might fit best or clarify what might be a challenge for you in a given specialty. Give it a try and let me know how it goes.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400\">If after going through that exercise you still find yourself staying up all night Googling <a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"https:\/\/www.google.com\/search?source=hp&amp;ei=0TFpWrT7B4jezwKV1an4BA&amp;q=how+to+choose+a+medical+specialty&amp;oq=how+to+choose+a+medical+specialty&amp;gs_l=psy-ab.3..0l7j0i22i30k1l3.754.754.0.1006.2.1.0.0.0.0.83.83.1.1.0....0...1c.2.64.psy-ab..1.1.82.0...0.2ChsAQPZmjQ\" target=\"_blank\">\u201cHow to choose a medical specialty\u201d<\/a> and are finding that everyone&#8217;s two cents are making your head spin, I recommend taking a step back and looking at the bigger picture. <\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400\">It was during this time in my own journey that I discovered the <a href=\"http:\/\/www.passionplanner.com\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">Passion Planner<\/a>, a book that can be ordered online (for every one purchased, one is donated) or is available as a <a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"http:\/\/www.passionplanner.com\/downloads\/\" target=\"_blank\">free download<\/a>. The planner was developed as a tool to help individuals clarify and define their goals and dreams and break them down into actionable steps. Whether you are looking for a day-to-day guide or just want to invest 15 minutes in a reflective activity, there is value in going through the Passion Roadmap exercise.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400\"><strong>The instructions are simple<\/strong>:<strong> <\/strong>set a timer for five minutes, and imagine that this piece of paper is magical\u2014whatever you write in it will come true within the designated time frame. Think of it as a wish list describing your ideal life. Ask yourself, <strong>\u201cIf I could be anything, do anything, or have anything, what would it be?\u201d<\/strong> Write every single thing that comes to mind and be as specific as possible. Lastly, do not feel the need to be realistic or justify your dreams. <\/span><strong>Just write<\/strong><span style=\"font-weight: 400\">.<\/span><\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"289\" height=\"269\" src=\"https:\/\/www.roshreview.com\/wp-content\/uploads\/Diagram.png\" alt=\"Wish list\" class=\"wp-image-2225\"\/><\/figure>\n<\/div>\n\n\n<p><span style=\"font-weight: 400\">The next step is to set a one-minute timer and circle the one goal in each of the four sections that would have the most positive impact on your life. Then, <strong>choose a \u201cgame changer<\/strong>,<strong>\u201d<\/strong> <\/span>the goal that would have <strong>the most positive impact on your life right now<\/strong><span style=\"font-weight: 400\">. Take five minutes to write down the steps you must take or tasks you must complete to reach your goal. Create a timeline using these details of what you need to do.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400\">Try it out, you might be surprised by what you find.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400\">When I did this activity at the height of my \u201cspecialty crisis,\u201d I was shocked to discover that what I needed most, my personal game changer (even more than envisioning myself in a given specialty), was to go on a camping trip and be entirely immersed in nature. Despite my love of mentorship and advice, I needed time and space away from all the noise, stress, and technology to reflect on my decisions without searching for my answer externally. Going on a camping trip turned out to be the catalyst that recentered me and allowed me to move forward in making confident decisions.<\/span><\/p>\n\n\n\n<h6 class=\"wp-block-heading\">Another great tools from the Passion Planner is a monthly reflection with six questions:<\/h6>\n\n\n\n<ol class=\"wp-block-list\">\n<li><span style=\"font-weight: 400\">What was the most <strong>memorable<\/strong> part of this past month? Describe it.<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400\">What were the three biggest <strong>lessons<\/strong> you\u2019ve learned this past month?<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400\">Review your past month and assess your <strong>priorities<\/strong>. Are you happy with how you spent your time? If not, what steps can you take this next month to adjust them?<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400\">How are you <strong>different<\/strong> between this past month and the month before?<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400\">What or who are you especially <strong>grateful<\/strong> for this past month?<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400\">Name three things you can <strong>improve<\/strong> on this upcoming month. What are concrete actions you can take to work towards these improvements?<\/span><\/li>\n<\/ol>\n\n\n\n<p><span style=\"font-weight: 400\">Answering these questions during times of uncertainty can be grounding and remind you that despite not knowing what the future may bring, we are not stagnant in the present and there are growth and opportunities for gratitude right now. I challenge you to give it a shot and see what you learn.<\/span><\/p>\n\n\n\n<p><span style=\"font-weight: 400\">I also encourage you to <strong>think broadly and creatively<\/strong> about your career path. If after evaluating all of the specialties you still find yourself stuck, give yourself the freedom to ask whether you want a career that is clinical vs. nonclinical. There are job opportunities for medical school graduates that may be a better fit for some people than becoming a clinician. While medical training is often a long and linear process, it certainly does not have to be that way. Examine your skills, talents, and interests and ask yourself how you might be able to combine them in a way that is most meaningful and fulfilling to you.<\/span><\/p>\n\n\n\n<hr class=\"wp-block-separator has-css-opacity\"\/>\n\n\n\n<p><span style=\"font-weight: 400\"><strong>As residency applications came out during my fourth year of medical school, I had to be honest with myself and acknowledge that it wasn\u2019t the right time to move in that direction<\/strong>. I decided to take a gap year and spend time delving into my passions that had been on the back burner during medical school. I expanded my knowledge and love of nutrition in a certification program that gave me the skills to work with my future patients to use food as medicine in both the management and reversal of chronic diseases. I then explored my interest in the media and public health by working as a medical producer of a TV show, which gave me insight into the behind-the-scenes aspects of how health information is produced.
